School-Based Services
Yuma Union High School District and Community Partners (CP) have partnered to provide school-based services (SBS). They are a referring agency that works with a child's family to address mental and behavioral health concerns. CP is an agency with professionals that can work with families to create an intervention plan for mental and behavioral needs. The school, family and CP will work closely together to support each individual child's needs. If you would like your child to be referred for Community Partners services, please contact your school’s Social Worker or School Counselor for more information and a referral form. Once enrolled with CP, each child is assigned an SBS interventionist that will coordinate care, facilitate Child and Family Team meetings, family support/resources and living skills as needed. If needed, an SBS therapist will be assigned who can facilitate therapy to help process mental health concerns.
About Jake's Law
S.B. 1523 is named in honor of Jake Machovsky, an Arizona teen who lost his life to suicide in 2016 after battling mental health issues.
In 2020, the Arizona legislature passed “Jake’s Law” which created a fund that provides uninsured and underinsured children access to behavioral health services when they are referred through an educational institution—otherwise known as “school-based referrals.” To make school-based referrals, the legislature required school districts and charter schools to pass a policy that outlined the law as it relates to school-based referrals. The policy must include:
- An opt-in process for parents
- A survey for parents whose children utilize school-based referrals
- A list of service providers published on the district website
- An annual report to the Arizona Health Care Cost Containment System (AHCCCS)
